Authorized Personnel Form
It is important that you submit a
new Authorized Personnel Form for
any changes for personnel who are
authorized to perform functions and
transactions for your financial
institution with the FHLB.
We cannot honor changes unless a new
form has been submitted. If you need
to update authorized personnel for eAdvantage, Advances, AHP Grants,
Collateral Services, Wire Transfers
and Safekeeping, please complete
this form.

Emergency Source of Funding -
Delivery of Advance Proceeds Form
The FHLB Des Moines has an
agreement with the Federal Home Loan
Bank of Pittsburgh (Pittsburgh Bank)
to offer advances to each other's
members in case one of the Banks is
unable to operate.
Under this agreement, the Pittsburgh
Bank will provide overnight advances
to Des Moines Bank members subject
to their financial and operational
ability to do so. Members will need
to provide the Des Moines Bank with
Federal Reserve Bank draw down
instructions to facilitate the
transfer of funds from the
Pittsburgh Bank to their
institution. If you do not have an
up-to-date Emergency Source of
Funding form on file, please
complete the form below specifying
whether you would issue draw down
requests on your own behalf or if
you would utilize a correspondent
bank.
As this information will be shared
with Pittsburgh Bank, it is
important for you to notify us if
you change correspondent banks.
