Subsidy Transfer Request
To draw funds for an Affordable Housing Program (AHP) grant award, please submit a Subsidy Transfer Request Form and any other documentation specified on the Subsidy Transfer Request checklist, including applicable sections of the Feasibility Review Workbook and project monitoring spreadsheets. For assistance call the Community Investment Department at 800.544.3452, ext. 1173.

This form must be signed by an authorized individual at the member bank.

Feasibility Review

Feasibility Review Workbook - Exhibit 1
Includes tabs for Exhibits I-A, I-B, III and IV. Sponsors should complete Exhibits III and IV for all units in the project to ensure the need for AHP funds and that the funded unit meets AHP feasibility guidelines. For development projects and down payment and closing cost assistance projects, please complete Exhibit I-A; for Habitat for Humanity projects, please complete Exhibit I-B.

Monitoring Spreadsheets by Project Type
Please provide information on completed units using one of the project spreadsheets below. Use the appropriate spreadsheet for the appropriate project type. These spreadsheets are for projects in the 2009A Round. If you require spreadsheets for earlier rounds contact the Community Investment Department at 800.544.3452, ext. 1173.

Development Projects
For projects including new construction and/or acquisition with rehabilitation for resale.


Habitat for Humanity Projects
For any development projects with developer financing for a homebuyer at or below market interest rate. Includes but is not limited to Habitat projects.

Retention Agreements (Deed Restrictions)
Each unit assisted in a homeownership project is subject to a five year deed restriction. The owner of each unit must enter into an agreement that will be recorded and filed in the appropriate jurisdiction by the member or sponsor.
